Sources: STEP 1 semaglutide DXA sub-study (n=140) — 10.4 kg of fat and 6.9 kg of lean mass lost, with lean mass rising as a share of body weight. SURMOUNT-1 tirzepatide body-composition sub-study (Look et al., Diabetes, Obesity and Metabolism, 2025) — roughly 75% of weight lost was fat and 25% lean, in both the tirzepatide and placebo groups. Lean mass loss accompanies rapid weight loss in general; it is not unique to the medication.

GLP-1s suppress appetite. That drives fast weight loss — but weight loss is never pure fat. In the SURMOUNT-1 body-composition sub-study, roughly one pound in four came off as lean mass — and the same held true in the placebo group. Losing lean mass isn't something the drug does to you. It's what fast weight loss does to anyone who isn't training through it.
What the medication adds is speed and a very small appetite — you're losing quickly, on far less food, which is exactly the condition where lean mass goes first. You weigh less but look softer. Strength drops. And lean tissue is metabolically expensive, so losing enough of it lowers your resting metabolism. Then you come off, appetite comes roaring back, and you're eating like you used to on a smaller engine. That's the regain trap.
The fix isn't stopping the medication. It's adding what the medication can't do: structured strength training to keep the muscle, plus enough protein to fuel it even when you can barely eat.
